March 5, 2013 REVENUE MEMORANDUM CIRCULAR NO. 024-13 SUBJECT : Lost Fifty (50) Unused Sets of Accountable Form No. 51 Official Receipt TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of fifty (50) unused sets of Accountable Form No. 51 Official Receipt, bearing the following serial numbers: AHEDaI ACF201000776651-ACF201000776700 The abovementioned forms were reported as lost by Ms. Fely I. Bonete, Revenue Officer I, Revenue District Office No. 53B, Revenue Region No. 8, Makati City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
