February 22, 2013 REVENUE MEMORANDUM CIRCULAR NO. 022-13 SUBJECT : Missing Duplicate Copy from One (1) Unused Set of BIR Form No. 0421 Tax Verification Notice TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing duplicate copy from one (1) unused set of BIR Form No. 0421 Tax Verification Notice (TVN) with the hereunder stated serial number, to wit: TVN200300158282 The abovementioned copy was reported missing by Ms. Maria Belen S. Pajarillo, Computer Operator I, Revenue District Office No. 82, Revenue Region No. 13, Cebu City. Consequently, the entire TVN set bearing the said serial number is hereby cancelled. All official transactions involving the use thereof is therefore considered as INVALID. HSCATc Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid form is found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
