March 29, 2011 REVENUE MEMORANDUM CIRCULAR NO. 016-11 SUBJECT : Lost Three (3) Unused Sets of Accountable Form No. 51 Official Receipt TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the loss of three (3) unused sets of Accountable Form No. 51 Official Receipt, bearing the following serial numbers: aSDCIE ACF201000164712 ACF201000164726 ACF201000164795 The abovementioned forms were reported as lost by Ms. Ma. Rowena I. Goda, Revenue Collection Officer, Revenue District Office No. 63, Revenue Region No. 9, San Pablo City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
