March 20, 1978 REVENUE MEMORANDUM CIRCULAR NO. 013-78 TO : All Internal Revenue Officers, Employees and Others Concerned SUBJECT : Loss of One (1) Set of Revenue Tax Receipt Bearing Serial Number 11401994 Notice is hereby given of the loss of one (1) set of Revenue Tax Receipt, to wit: Serial Number Quantity 11401994 One (1) set The above Revenue Tax Receipt was reported missing by JUANA B. CABRERA, ALFONSO S. SARMIENTO, and DONATA M. ROLA, Revenue Collector II, Revenue Collector I, and Cash Clerk, respectively of Revenue District Office No. 43, San Jose, Occidental Mindoro.
