December 12, 2007 REVENUE MEMORANDUM CIRCULAR NO. 011-08 SUBJECT : Loss of Two (2) Original Copies of Tax Credit Certificates Issued to Benguet Center Bank, Inc. TO : All Regional Directors, Revenue District Officers, Employees and Others Concerned Notice is hereby given of the loss of two (2) original copies of Tax Credit Certificates (TCCs) to wit: SERIAL NUMBER DATE OF ISSUE AMOUNT TCC 2006 00002748 May 24, 2007 Php247,432.34 TCC 2006 00002749 May 24, 2007 Php226,677.16 The above-mentioned TCCs were reported as lost by Mr. ELMO B. BELA-O, President and Chief Operating Officer of Benguet Center Bank, Inc. Consequently, the entire TCC set bearing the said serial numbers are hereby cancelled. All official transactions involving the use of said form is therefore considered as INVALID. Internal Revenue Officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCCs are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
