December 28, 2000 REVENUE MEMORANDUM CIRCULAR NO. 01-01 SUBJECT : Loss of Thirty Seven (37) Sets of Unused BIR Form No. 25.24 Revenue Official Receipt TO : All Internal Revenue Officers, Employees and others Concerned Notice is hereby given of the loss of Thirty Seven (37) sets of unused BIR Form No. 25.24 Revenue Official Receipt, to wit: Serial Number Quantity ROR 199900295314 - ROR 199900295350 37 sets (unused) The abovementioned forms which were reported as lost by MS. MA. SALOME B. PEROS, Revenue Collection Officer of the Municipality of Balangkayan, Revenue District Office No. 86, Eastern Samar, have been cancelled and all official transactions involving the use thereof are therefore invalidated. Internal Revenue Officers, employees and others concerned are requested to promptly notify this Office in case the abovementioned copies are found, taking the necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof.
