(Now Pilmico Animal Nutrition Corporation) TO : All Regional Directors, Revenue District Officers, Employees and Others Concerned Notice is hereby given for the loss of one (1) original copy of Tax Credit Certificate (TCC) to wit: DaTISc TCC No./Serial No. Date of Issue TCC Face Amount TCC Balance 200600000154 April 19, 2007 P40,000,000.00 P8,000,000.00 The above-mentioned TCC was reported as lost by IVY ELEANOR T. UY, Finance Manager, Pilmico Animal Nutrition Corporation on October 27, 2009. Consequently, the entire set bearing the said TCC No. is hereby cancelled. All official transactions involving the use of the said form effective October 28, 2009 is therefore considered as INVALID. Internal revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid TCC is found and to undertake necessary measures or actions in order to prevent the improper or fraudulent disposition/use of the same.
