December 29, 2009 REVENUE MEMORANDUM CIRCULAR NO. 004-10 SUBJECT : Missing Five (5) Pads of an Unused Accountable Forms No. 51-Official Receipt TO : All Internal Revenue Officials, Employees and Others Concerned Notice is hereby given of the missing five (5) pads of an unused Accountable Form No. 51-Official Receipt, bearing the following serial numbers: ACF200500528001-ACF200500528250 The abovementioned forms were reported as missing by Ms. Aileen R. Perez, Finance Officer I, Revenue Region No. 10, Legazpi City, and have consequently been cancelled. All official transactions involving the use of said forms are therefore considered as INVALID. Internal Revenue officials, employees and others concerned are requested to promptly notify this Office in the event that the aforesaid forms are found and to take the necessary measures to prevent the improper or fraudulent disposition or use of the same.
