December 18, 1978 REVENUE MEMORANDUM CIRCULAR NO. 003-79 TO : All Internal Revenue Officers, Employees and others concerned SUBJECT : Loss of One (1) Set of Revenue Tax Receipt (CB RCO Form No. 02.02) bearing Serial Number 2466847 Notice is hereby given of the loss of one (1) set of Revenue Tax Receipt, to wit: Serial Number Quantity 2466847 One (1) Set The above Revenue Tax Receipts were reported to the Regional Director, Revenue Region No. 4-C, San Pablo City by ALFONSO S. SARMIENTO, Revenue Collection Agent, San Jose, Occidental Mindoro as missing from the booklet. Internal revenue officers, employees and others concerned are requested to promptly notify this Office in case the above-mentioned Revenue Tax Receipt is found, taking necessary steps in the meantime to prevent the improper or fraudulent disposition or use thereof. cdt EFREN I.
