March 31, 2004 REVENUE DELEGATION AUTHORITY ORDER NO. 01-04 SUBJECT : Delegation of Authority to File BIR Dues on Forms 1600 Monthly Remittance Return of Value-Added Tax and Other Percentage Taxes Withheld, 1604CF Annual Information Return of Income Taxes Withheld on Compensation and Final Withholding Taxes, 1604E Annual Information Return of Creditable Tax (with their Corresponding Attachment); 1601C Monthly Remittance Return of Income Taxes Withheld on Compensation, 1601F Monthly Remittance Return of Final Income Taxes Withheld and 1601E Monthly Remittance Return of Creditable Income Taxes Withheld (Expanded) thru Electronic Filing and Payment System TO : All Concerned Offices This order is being issued to ensure continuity in the discharge of official functions and grant access to authorized personnel to the Electronic Filing and Payment System (eFPS). The following revenue officials and personnel are hereby delegated with the authority to file BIR Forms: 1600, 1604CF and 1604E (with their corresponding attachment); 1601C, 1601F, and 1601E via eFPS. Alquiros Computer Operator - Felix V. Villaflor Revenue Officer E. Finance Division - Evelyn C. de Guzman Division Chief RR 8 Makati - Fely I. Bonette Fiscal Examiner - Mary Grace C.
