Edna A. David, Hon. Monina P. Laus BE IT ORDAINED BY THE SANGGUNIANG PANLALAWIGAN OF PAMPANGA, in session assembled, that: SECTION 1. The title, paragraphs 2 & 4 of Section 2 and Section 3 of Tax Ordinance No. 01, series of 2009 are hereby amended, to read as follows: "AN ORDINANCE IMPOSING AFFILIATION FEE AGAINST NURSING STUDENTS RENDERING ON THE JOB TRAINING DUTY AT THE DIFFERENT PROVINCIALLY FUNDED HOSPITALS OF THE PROVINCE; AND FOR OTHER PURPOSES. HTcADC BE IT ORDAINED BY THE SANGGUNIANG PANLALAWIGAN OF PAMPANGA, in session assembled, that: Section 1. There is hereby imposed an affiliation fee of One Hundred Fifty (Php150.00) Pesos against each nursing student rendering on the Job Training Duty commensurating to sixty (60) hours up to eighty (80) hours of service at any provincially funded hospital of the Province of Pampanga from which accredited medical or nursing institutions shall shoulder the amount so assessed for the purpose. Section 2. That the proceeds derived therefrom shall accrue to the fund of the province and concerned provincially funded hospitals distributed as follows: 2.1. Five (5%) percent to the Province; 2.2. Forty (40%) percent to the concerned hospitals for the acquisition of supplies and materials and other equipment; and 2.3. Fifty Five (55%) percent shall also accrue to the concerned hospitals to be earmarked as follows: 2.3.1. Sixty (60%) percent to be allotted for Training Services; 2.3.2. Forty (40%) percent to be allotted for remuneration of other hospital personnel. Provided, however, that one (1%) percent of the amount allotted for training services shall be set aside for the remuneration equally divided among the administrative staff of the Provincial Health Office including the Provincial Health Officer II and the remaining balance of ninety nine (99%) percent shall be allotted as follows: a. Forty (40%) percent for additional remuneration equally divide among the incumbents of Nurse II, Nurse III and Nurse IV; b. Ten (10%) percent for the additional remuneration of the incumbent Chief of Hospital; and c. Fifty (50%) percent for the additional remuneration of the incumbent Nurse I. Provided, further, that the concerned Chief of Hospital may, at his sound discretion, authorize the disbursement of funds allotted for such purposes. Provided, furthermore, that the proceeds of affiliation fees paid on or before this imposition which were duly covered and embraced by Contract of Affiliation shall be acknowledged accordingly and be likewise applied for the purposes as authorized in the preceding paragraphs hereof.
