July 25, 2001 NAGA CITY ORDINANCE NO. 048-01 AN ORDINANCE FURTHER AMENDING ARTICLE B & C OF CHAPTER IV OF ORDINANCE NO. 543, AS AMENDED BY ORDINANCE NO. 88-021, AND SECTION 9 OF ORDINANCE NO. 93-060, OTHERWISE KNOWN AS "THE LOCAL REVENUE CODE OF NAGA CITY" Authors: Hon. John G. Bongat Hon. Jose L. Grageda Be it ordained by the Sangguniang Panlungsod of the City of Naga, that: SECTION 1. Article B & C of Chapter IV of Ordinance No. 543, otherwise known as "The Revenue Code of the City of Naga" as amended by Ordinance No. 88-021 is hereby further amended, now to read as follows: "ARTICLE B Civil Registry Fees and Permit Fees for Cadaver Disposition "SEC. 4B.01. Imposing of Fees. There shall be collected the following fees for the services rendered by the Civil Registry Office of Naga City: a. For the registration of documents and for certified copies of documents on file in the City Civil Registry: DIESHT 11. Late registration of birth, marriage, and death (Subject to the provisions of Sections 2 & 5 of P.D. No. 651, as amended by P.D. No. 766 dated August 8, 1975): Beyond thirty (30) days but not exceeding P100.00 one (1) Year 12. Late registration of birth, marriage, and 200.00 death beyond one year 13. Certified true copy or certified xerox copy 200.00 of: a. birth certificate for passport application b. birth certificate for ordinary use 20.00 c. marriage certificate 30.00 14. Certified true copy or certified xerox copy of 20.00 death certificate" SECTION 2. Section 19 of Ordinance No. 93-060 is hereby amended, now to read as follows: "SEC. 19. Civil Registry Fees and Permit Fees for Cadaver Disposition. There shall be collected the following fees for services rendered by the Civil Registry of Naga City: A. 1. Legal Separation P3,000.00 2. Annulment of Marriage 3,000.00 3. Court decision or order to correct or change of 250.00 entries in any certificate of birth 4. Voluntary emancipation of minors 250.00 5. Repatriation or voluntary renunciation of citizenship 250.00 6. Court decision recognizing or acknowledging of natural 250.00 children or impugning or denying such recognition or acknowledgment 7. Judicial determination of maternity affiliation 250.00 8. Aliases 100.00 9. Court decisions or orders to the custody of guardianship 250.00 or adoption 10. Filing of supplemental birth certificate to furnish 100.00 additional data 11. Endorsement to NSO fee (n) 100.00 B. For other entries in the registry: 1. For registration of any legal document for Record purposes 200.00 2. For registration of affidavit and/or oath of allegiance 200.00 regarding wives and children of naturalized Filipino citizen or election of citizenship 3. For registration of foreign decree of Adoption 200.00 C. Marriage License and Solemnization Fees 1. . . . 2. Solemnization fee when the marriage is performed 100.00 by any of the judges of the City Courts or the City Mayor D. Permits for Cadaver Disposition: 1. Burial Permit Fee 50.00 2. For the disinterment/exhumation of human Remains 100.00 3. Removal of Cadaver 100.00 4. Entrance of Cadaver from another City/Municipality 100.00 5. Transfer of Cadaver from Naga City to another 100.00 city/municipality" SECTION 3. Section 20 is hereby added thereto, now to read as follows: "SEC. 20. Fee for Issuance of Security Paper (SECPA). There shall be collected a service fee of P200.00 for every issuance of certification done in SECPA (n). ARTICLE C Administrative Proceedings Pursuant to Republic Act No. 9048" EHSTDA SECTION 4. Fees and Charges for Filing Petition for Change of First Name or Nickname and/or Correction of Entries as mandated under Republic Act No. 9048 and its Implementing Rules and Regulations. A) There shall be collected the following fees and charges, to wit: 1. Filing fee for Petition for Change of First Name or Nickname P3,000.00 2. Filing fee for petition for correction of Entry 1,000.00 3. Service Fee for Out-of-Town Reporting 500.00 SECTION 5. Trust Fund. There shall be set aside Twenty (20%) per cent of every filing fee or charges collected which shall form as a trust fund of the City Civil Registrar's Office to be used solely for the operational expenses of the administrative proceedings (n). SECTION 6. Separability Clause. All orders, resolutions, ordinances, rules and regulations or parts thereof which are inconsistent with any provision of this Ordinance are hereby amended, repealed and/or modified accordingly; SECTION 7. Effectivity. This ordinance shall take effect upon its approval and after publication in a newspaper of general circulation. ENACTED: July 25, 2001.
