March 6, 2013 MINA ORDINANCE NO. 233-13 AN ORDINANCE AMENDING ORDINANCE NO. 2012-212 OTHERWISE KNOWN AS REVISED MUNICIPAL REVENUE CODE PARTICULARLY ON CHAPTER IV, ARTICLE B, ADDITIONAL FEES ON LOCAL REGISTRY OF THE MUNICIPALITY OF MINA Be it ordained by the Sangguniang Bayan, in session assembled that: SECTION 1.01. Imposition of Fees. There shall be an amendment in the collection of fees for services rendered by the Local Civil Registrar of the Municipality, to wit: a. Electronic Endorsement/Advance Endorsement P75.00 b. Certified True Copy of Birth, Death & Marriage Certificate (new form) P75.00 c. Certified Machine Copy from the Original File/Certified Transcription Copy P50.00 d. Change of Surname under RA 9255/Reg. of AUSF P50.00 e. Registration of Affidavit of Acknowledgement/Paternity P50.00 f. Certification Fee P50.00 SECTION 3F.03. Time of Payment. The fees shall be paid to the Municipal Treasurer's Office at the time of the request, written or otherwise, for the issuance of the copy of any municipal record or document is made. SECTION 4. Effectivity Clause. This Ordinance shall take effect upon approval and after publication. ADOPTED this 6th day of March 2013, on motion Hon. Pedro P. Mana-ay and was unanimously seconded.
