RESOLUTION NO. 433, S. 2011 (Authored by: Councilor Noel C. de Jesus) Taken up were the recommended fees submitted by the City Health Officer for affiliation fee for nursing and allied services students, and the use of NUGA BEST equipment. cEaSHC A perusal of the affiliation fee showed that the same is consistent with the guidelines issued by the Department of Health (DOH) Administrative Order No . 5-A, S. 1996 . In view thereof, on motion of Councilor Noel C. de Jesus , duly seconded by Councilors Esperidion V. Catan , Nilo D. Sayson and Franklin D. Esmea, Jr ., the Sangguniang Panlungsod in session assembled RESOLVED, AS IT IS HEREBY RESOLVED, to enact the following ordinance: DUMAGUETE CITY ORDINANCE NO. 219-11 AN ORDINANCE FURTHER AMENDING ORDI NANCE NO. 139, SERI ES OF 2008, ENTITLED "THE UPDATED OMNIBUS TAX ORDINANCE OF THE CITY OF DUMAGUETE," SPECIFICALLY SECTION 69 (H)-II. CITY HEALTH ANNEX BIRTHING SERVICE FEE Be it ordained by the Sangguniang Panlungsod of the City of Dumaguete, that : SECTION 1. Letter (q) Affiliation fee for Nursing and Allied Services Students under Item No. 22 , Section 69 (h)-II. City Health Annex Birthing Service Fee of Ordinance No. 139, series of 2008 , as amended by Ordinance No. 105, Series of 2009 , is hereby further amended by changing its fee, to read: "Section 69 (h). Sanitary Inspection and Health Certificate Fees. x x x (I. Sanitary Inspection Fee) II. City Health Annex Birthing Service Fee : x x x [1 to 22-(a) to (q)] q) Affiliation fee for Nursing and Allied Services for Students P50.00/student for 1-29 hrs. P80.00/student for 30-49 hrs. P120.00/student for 50-80 hrs." SECTION 2. Section 69 (h)-II. City Health Annex Birthing Service Fee of Ordinance No. 139, series of 2008 , as amended by Ordinance No. 105, Series of 2009 , is hereby further amended by adding Wellness Clinic Fee , particularly the use of NUGA equipment to be assigned as No. 24 , to read: IAETDc x x x [1 to 23. Wellness Clinic Fee (NUGA BEST equipment) : a) NUGA Bed P50.00/40 mins. b) Diet Pad P25.00/40 mins. c) Bio-Heart Foot Reflexors P25.00/12 mins. SECTION 3. Effectivity . This ordinance shall take effect after certified true copies of this ordinance are published in full for three (3) consecutive days in a newspaper of general circulation in the Province of Negros Oriental.
