November 6, 2007 DOF DEPARTMENT ORDER NO. 042-07 FOR : All Officials and Employees of This Department, Bureaus and Attached Agencies SUBJECT : Documentary Requirements for Personal Travel Abroad It has been observed that despite previous memoranda on request for authority to travel abroad (Personal), many requests for travel authority cannot be favorably acted upon for reason that documentary requirements have not been fully complied. EcTCAD To facilitate approval of personal travel request, the following documents should be attached: a. Letter request of the concerned official/employee recommended by Head of Office/Bureau/Agency; b. Application for Leave for the period covered duly approved by authorities concerned; c. Clearance duly signed by the responsible official that the applicant has no pending administrative case; d. Sworn statement of the applicant, attested by the recommending official, stating: i.) the estimated cost trip; ii.) how the trip will be financed; iii.) if on his own account, the total income of the applicant for the preceding tax year and the amount of income tax paid; iv.) if for the account of other person/s, the income earned by such person/s for the preceding tax year and the income tax paid, to include Philippine or foreign income tax. The above documentary requirements shall be submitted to this Department, otherwise no action shall be taken on incomplete supporting documents. This Department Order supersedes MC No. 7-84 dated September 4, 1984. This Order shall take effect immediately. (SGD.) MARGARITO B. TEVES Secretary
