February 12, 2015 DOF DEPARTMENT ORDER NO. 019-15 SUBJECT : Designation of Signing Authority for the Office of the Director III, Central Administration Office In the exigency of the service and in view of the retirement of Director IV LOURDES Z. SANTIAGO of the Central Administration Office end-of-day February 11, 2015, JENNIFER J. TAN, DIRECTOR III of Central Administration Office is hereby designated to take over Director Santiago's immediate administrative functions/authorities including that of approval and signatory as stated in Department Order No. 30-2012 issued September 11, 2012 ; "1. Approval and signatory of: 1.1 Requisition and Issue Slip/Request for Purchase/Service Quotation; 1.2 Application for Retirement Gratuity to GSIS; 1.3 Leave applications regardless of rank; 1.4 Report of Waste Materials; 1.5 Various administrative issuances to be disseminated; 1.6 GSIS Membership Form of new employees; 1.7 Communications relative to decisions of the Office of the Ombudsman; 1.8 Travel authority (both official and personal) below the rank of Directors in the absence of Undersecretary Gil S. Beltran and Assistant Secretary Ma. Teresa S. Habitan; and 1.9 Department Personnel Orders for local travel and attendance in local seminars, workshops, and other training programs in the absence of Undersecretary Gil S. Beltran and Assistant Secretary Ma. Teresa S. Habitan; and 2. Recommends approval of clearance of DOF officials and employees from money, property and other accountabilities; 3. Recommends approval of Disposal Committee Resolution; 4. Certifies the following accountable forms and documents: 4.1 Employment, Compensation and Benefits; 4.2 Position Description Form; 4.3 Service Record; 4.4 Payroll and other claims for benefits; 4.5 Last salary received of concerned personnel; 4.6 Accumulated leave credits. cTAaDC 5. Alternate signatory for the following accountable forms/documents in the absence of Assistant Secretary Ma. Teresa S. Habitan, Director Maria Estela C. Laureno and Director Ma. Lourdes V. Dedal: 5.1 Disbursement vouchers regardless of the amount; 5.2 Counter signatory for checks issued and Advice of Checks Issued and Cancelled (ACIC) issued by the Department regardless of the amount; 5.3 Debit Memo." All orders, memoranda, circulars or other issuances or parts thereof which are inconsistent with this Department Order are hereby deemed repealed and/or modified accordingly. This Department Order shall take effect on February 12, 2015 and shall be valid until sooner revoked, or until the appointment of a new Director IV, Central Administration Office. (SGD.) CESAR V.
