Internal Guidelines in the Availment of Medical and Dental Services

March 5, 2007 DOF DEPARTMENT ORDER NO. 008-07 TO : All Officials and Employees This Department SUBJECT : Internal Guidelines in the Availment of Medical and Dental Services Article X of the Collective Negotiation Agreement (CNA) between the Department of Finance and the Department of Finance (Proper) Employees Association (DOFPEA), provides for Medical and Dental Services for the officials and employees of this Department. Accordingly, the following guidelines shall be observed: 1. COVERAGE: A. Bonafide employees of the Department of Finance and their dependents shall be eligible to avail of the Medical and Dental services of this Department. Dependents shall refer to the following: If Single: Both parents; or 2 siblings below 21 years old (if parents are deceased) If Married: Spouse Children If Married with no children: Both parents CAIHTE To augment their take home pay, the employees and their dependents shall be entitled to free consultation and free medicine. They shall be given the complete dosage of medicines for seven (7) days as prescribed by the Medical Officer, subject to the availability of funds. For this purpose, the Personnel Services Division shall provide the Medical Clinic with the list of employees and their dependents. 2. n No out patient except those identified in B and C below shall be accommodated by the Medical and Dental Clinic. 3. DOF personnel shall be entitled to the Annual Physical Check Up including general dental cleaning. 4. Medical Certificate shall be issued to prospective employee provided he has passed the physical examination, X-Ray, Drug Test, Blood Tying, Urinalysis, Hepa B and Hepa C Screening. 5. In case of sickness, the Medical Clinic, aside from the Personnel Services Division, Shall be informed of the absence of the employee as basis for validation of medical certificate. The Notification Form prescribed under Department Order No. 34-06 shall be used. 6. Medical Certificate coming from the private Physician must be submitted to the Medical Clinic using the letterhead of the private Medical Clinic indicating therein the License Number of the attending physician. 7. Home visit to employees with long absence due to sickness shall be done by the Medical Officer regularly, provided a vehicle shall be made available for the purpose. 8. Request for issuance of medical certificate for scholarship purposes shall be submitted to Medical Clinic together with results of required laboratory examinations at least five (5) days before deadline of sponsoring agency. 9. Employees requesting follow up prescription must present to the Medical Officer the previous prescription from their attending Physician. 10. Disbursing Officer of this Department and Attached Agencies needing medical certificate for issuance of Fidelity Bonds must present themselves to the Medical Officer for physical examination. 11. Free Flu vaccine shall be administered to all employees of the Department every year while the anti-pneumonia vaccine shall be administered every two (2) years, subject to availability of funds. DETACa 12. Dispensation of medicine shall be made only upon orders of the Medical Officer. 13. Except for Department employees and their dependents, those who will submit themselves to tooth extraction shall sign a waiver that the Dentist shall not be held liable. B. The following shall be entitled to free consultation only, except in emergency cases where the immediate medical attention and medicine may be given: 1. Outsourced personnel 2. Non-DOF personnel detailed with this Department 3. DOF Consultants 4. Janitors 5. Security Guards C. Personnel of the following government agencies within the premises shall likewise be entitled to free medical consultation only, except in emergency cases where the immediate medical attention and medicine may be given: 1. Bureau of Local Government Finance 2. Central Board of Assessment Appeals 3. CBRMP/LOGOFIND 4. Commission on Audit 5. Land Bank of the Philippines, BSP Servicing Unit 6. Build Operate Transfer (BOT) Center 7. Insurance Commission This Order takes effect immediately and all concerned shall be guided accordingly. (SGD.) MARGARITO B.
