February 4, 2011 DOF DEPARTMENT ORDER NO. 005-11 ( Declared Superseded by D.O. 29-16) In the exigency of the service, the Committee on Rewards is hereby reconstituted as follows: Chairperson : Undersecretary Carlo A. Carag Revenue Operations & Legal Affairs Group Vice Chair : Undersecretary Jeremias N. Paul, Jr. Corporate Affairs Group Members : Assistant Secretary Peter L. Calimag Revenue Operations & Legal Affairs Group Director Ma. Lourdes V. Dedal Central Financial Management Office Director Lourdes Z.
