January 16, 2006 DOF DEPARTMENT ORDER NO. 004-06 RECONSTITUTION OF THE BOARD OF PERSONNEL INQUIRY AND REVIEW (BOPIR) In the exigency of the service, the Board of Personnel Inquiry and Review (BOPIR) is hereby reconstituted and shall be composed of the following: AHDacC Undersecretary Gaudencio A. Mendoza, Jr. - Chairman Assistant Secretary Carlito P. Yu - Member Director Thelma A. Mariano - Member Director Lourdes Z. Santiago - Member Director Ma. Lourdes V. Dedal - Member Director Ma. Teresa S. Habitan - Member The Legal Affairs Office shall act as its Secretariat. This Order shall take effect immediately and all concerned shall be guided accordingly. (SGD.) MARGARITO B.
