March 13, 2013 BLGF MEMORANDUM CIRCULAR NO. 040-13 TO : All Bureau Officials and Personnel; Regional Directors for Local Government Finance; Provincial, City and Municipal Treasurers and Others Concerned SUBJECT : Affidavit of Disclosure In connection with the forthcoming May 13, 2013 national and local elections, all provincial, city and municipal treasurers are hereby requested to accomplish and submit to this Bureau, thru the Office of the Regional Director for Local Government Finance, on or before April 12, 2013, an Affidavit of Disclosure (Sample form Attached) indicating therein whether or not they have relatives within the fourth civil degree of consanguinity or affinity of any candidate in their respective official stations. The Provincial Treasurers are responsible in the distribution and retrieval of the duly accomplished Affidavit of Disclosure from their respective Municipal Treasurers, including City Treasurers, if any, for consolidation and submission to this Bureau on or before the set deadline, enclosing therein said affidavits of all treasurers under their jurisdictions.
