January 18, 2006 BIR MEMORANDUM TO : All Revenue Officials Concerned SUBJECT : Issuance of Tax Verification Notice Forms Notice is hereby given that Tax Verification Notice (BIR Form No. 0421) shall only be issued by the Accountable Forms Division (AFD) in sufficient quantities not to exceed three months' requirement of the requisitioning BIR office. Moreover, further issuance of said accountable form shall be made only upon consumption of previously issued TVNs as evidenced by the Report of Accountability for Accountable Forms and Stock Position Sheet to be submitted to the Accountable Forms Division by the Regional Office concerned.
