Manner of Disposal of BIR Form 17.58 (Authority to Cancel Assessment)

April 25, 2007 BIR MEMORANDUM TO : All Regional Directors Bureau of Internal Revenue This refers to the memorandum issued by the OIC-Assistant Commissioner of the Taxpayer Assistance Service stating, among others, that BIR Form No. 17.58 (Authority to Cancel Assessment) can be declared obsolete. In view thereof, you are directed to cause the disposal of said accountable forms in such manner as hereunder stated, to wit: a. Convene the Committee on Disposal of Accountable Forms in that Region to appraise and conduct an inventory of the abovementioned obsolete accountable forms (Please refer to RMO 86-99); b. Prepare and sign the inventory report and submit the same together with the request for Authority to Dispose to the OIC, Assistant Commissioner, Financial and Administrative Service; c. Upon approval of the Authority to Dispose, the Committee shall proceed with the disposal of the obsolete accountable forms in accordance with existing COA rules and regulations; CSIcHA d. The Committee shall prepare and submit a report with a certified true copy of the Certificate of Destruction to the Deputy Commissioner, RMG, Attention: The Chief, Accountable Forms Division, after completion of every destruction by burning or shredding of the obsolete accountable forms stated in the approved authority. aCSTDc Please be guided accordingly.
